Your initial appointment will consist of an examination, any needed x-rays and a review of your treatment options. Usually a teeth cleaning is also done on the same day, depending on your needs and time frame. We allow extra time in our schedule at that first appointment to make sure we get an opportunity to understand your goals and desires, and you get an opportunity to get to know us and ask questions, so that we can coordinate a treatment plan tailored to your needs.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card and fill out our online registration and health history forms. This will save time and allow us to help you process any claims. If you prefer, feel free to come 15 minutes early to your appointment and you can complete the forms in our office.
- Electronic forms are avaiable on this website to save you time.
